What is a medical recommendation card?

A medical recommendation card is a physical card that you can carry with you that proves that you have been seen by a doctor and recommended for the use of medical products legally. The card will have your name, photograph, and the doctor’s signature.

In order to get a medical recommendation card, you must first be seen by a licensed physician who will then fill out an application for you. Once this is complete, you will submit it to the Oklahoma State Department of Health (OSDH) for approval.

You are also required to pay a $25 fee every year. After all necessary information has been submitted, the OSDH will review the application and make their decision within 15 days. If approved, they will mail you your official identification card along with other materials so that you may apply at dispensaries across Oklahoma.

 

Who can apply for a medical recommendation card?

You must be a resident of Oklahoma and have a valid form of identification. You will need to fill out an application, which you can get from the Oklahoma Department of Health. You will also need to provide proof of residency, such as a driver’s license or utility bill.
